Dextromethorphan Hydrobromide / Benzocaine

Pronunciation: (DEX-troe-meth-OR-fan HIGH-droe-BROE-mide/BEN-zoe-caine)Class: Nonnarcotic antitussive

Trade Names:Cepacol Dual Relief- Spray 5 mg dextromethorphan and 5% benzocaine

Trade Names:Cepacol Ultra Sore Throat Plus Cough- Lozenges 5 mg dextromethorphan and 7.5 mg benzocaine

Trade Names:Tetra-Formula- Lozenges 10 mg dextromethorphan and 15 mg benzocaine


Suppresses cough by central action on cough center in medulla.

Indications and Usage

Temporary relief of minor sore throat; temporary reduction in cough caused by minor throat and bronchial irritation.


MAOI therapy or for 2 wk after stopping MAOI therapy; allergy to local anesthetics (eg, butacaine, procaine) or any component of this product.

Dosage and Administration

CepacolAdults and Children 12 yr of age and older

PO 2 sprays every 4 h (max, 8 sprays/24 h).


Allow 2 lozenges (1 immediately after the other) to dissolve slowly in the mouth; may repeat every 4 h (max, 12 per 24 h).

Children (6 to 12 yr of age)

PO 1 spray every 4 h (max, 4 sprays/24 h).


Allow 1 lozenge to dissolve slowly in the mouth; may repeat every 4 h (max, 6 per 24 h).

Tetra-FormulaAdults and children 6 yr of age and older

PO Dissolve 1 lozenge slowly in the mouth, do not chew. May be repeated every 4 h or as directed by health care provider.

General Advice

  • Cepacol Dual Relief
  • Spray into throat and then swallow.


Store at 59° to 86°F. Protect from moisture. Do not freeze.

Drug Interactions

MAOIs (eg, isocarboxazid)

Dextromethorphan is contraindicated with MAOIs.

Laboratory Test Interactions

None well documented.

Adverse Reactions


Drowsiness; dizziness.

Do not use Cepacol in children younger than 6 yr of age.

Chronic cough

Do not use in chronic cough such as occurs with smoking, emphysema, or asthma.

Excessive phlegm

Do not use with a cough that is accompanied by excessive phlegm.



Respiratory depression.

Patient Information

  • Advise patient to take as directed on the package insert or as advised by their health care provider.
  • Advise patient that if symptoms are not controlled not to increase the dose of medication or frequency of use but to inform their health care provider.
  • Instruct patient to notify their health care provider if any of the following occur: cough persists for longer than 1 wk or tends to recur; severe sore throat persists for more than 2 days, or if cough or sore throat is accompanied by fever, persistent headache, rash, swelling, nausea, or vomiting.
